Your Family’s Growth & Opportunities (GO) Plan

As your child’s parent or caregiver, you know your child best. You see their strengths, their interests, and the little changes in their development every day. Because of that, you play the most important role in helping us understand how to support your child.

Your Primary Partner will work closely with you to create an Individual Family Plan, co-designed with families for families. We call this our Growth and Opportunity Plan—or GO Plan.

Together, we’ll explore the support that can help your child grow, which may include services such as speech and language therapy, occupational therapy, social work, or psychology.

If Other Supports Are a Better Fit

If we find that your child’s needs would be better supported by another community service, we will help you every step of the way. With your consent, we will connect you directly—using a warm handoff—to ensure a smooth and seamless transition.
